1. Conduct needs assessments on each participant.
2. Counsel program participants through individual, family and group interaction.
3. Assist participants in the establishment of individualized housing stabilizing plans and monitoring of goals;
4. Connect participants to appropriate resources to promote improvement of skills.
5. Participate in continuous evaluation and improvement of program participant services.
6. Identify barriers to obtaining/maintaining permanent housing faced by program participants and ensure that participants obtain needed resources, including financial resources available through program and other sources, and resources relating to counseling, education, employment, etc. likely to enable program participants to obtain/maintain permanent housing.
7. Coordinate activities of outreach specialist.
8. Provided temporary financial assistance as needed.
9. Assist participants in obtaining VA benefits, other public benefits.
10. Maintain agency record keeping and all communications in a confidential matter as per agency and program policy.
11. Maintain and update case notes in an efficient and confidential manner.
12. Provide data and reports periodically or as requested by Director or lead agency by deadline dates.
13. Ensures that participant files include required eligibility documentation including veteran's status, income status, and housing status.
14. Ensures safety and security of program participants, staff and others according to agency policies.
15. Conducts and documents recertification of program eligibility when required.
16. Develops program policies and procedures.
17. Consistently and diplomatically enforce agency and program rules and policies.
18. Honor worksite policies when working as a guest at a rural satellite site.
19. Maintain work space in a professional and welcoming manner, including at host sites.
20. Provide information via telephone, make referrals, collaborate with the VA and other partner agencies, relationships with service providers and agencies in the communities are nurtured and maintained.
21. Maintains a complete case file with required documents.
22. Attend staff meetings and work assignment reviews regularly as scheduled.
23. Apprise staff of any program participant issues that potentially present a danger to program participant, other program participants, staff or others.
24. Maintain tracking of program participants at required appointments.
25. Attend trainings as required.
26. Represent the agency and program at community events, committees, or meetings as assigned.


Bachelor degree in psychology, sociology, social work, counseling or related field; with a minimum of three years of experience in case management, social services, homeless or related services.

Staff serving clients with brain injury must also receive training on that topic prior to service provision to members.

Knowledge of community resources in Dubuque and Delaware counties and experience working with Veterans a plus.

Must successfully complete child/dependent adult abuse and criminal background check, including DHS evaluation.

Must have experience working with persons experiencing homelessness, and/or persons with multiple needs such as mental health, substance abuse, financial, life skills, employment, etc.

Must be able to perform the essential functions of this position and complete job trainings within 30 days of hire.

Eligible agency driver required.
