Job Purpose:  Provides community-based mental health assessments, interventions, and consultation to individuals, providing crisis support, integration with the legal system, and ongoing connection with supportive resources.


Regulatory Requirements:

  • Currently licensed as a Social Worker, Mental Health Counselor or Marriage and Family Therapist or able to be independently licensed in Iowa within 6 months of hire.
  • Must be able to visually observe patient/client needs.
  • Must complete Mandatory Reporter for Child/Dependent Adult Abuse training within four months of hire date.
  • Must successfully complete child/dependent adult abuse and criminal background check, including DHS evaluation (if needed, including documentation of follow-through for any stated employment restrictions); reference check forms (minimum of two); and employment physical, TB test and drug screen prior to contact with clients and as required thereafter.


Demonstrates Competency in the Following Areas:

  1. Displays effective interviewing skills and good organizational skills in order to create professional and coherent writing when completing progress notes, forms, letters, and other necessary documentation.
  2. Attends and participates in Eastern Iowa Care Coordination Model training, quality monitoring monthly conference calls, and engages in as needed care coordination meetings to address high utilizers or complicated case review as appropriate.
  3. Provides community-based mental health assessments, interventions, and consultation to individuals identified by the court system as in need of a potential mental health committal.
  4. Practices within the defined scope and responsibilities of the Eastern Iowa Behavioral Health Care Coordinator Model.
  5. Provides direct mental health services to identified individuals incarcerated within the Jackson County jail and/or other client populations as identified by the MHDS Region.
  6. Follows individuals for a 30 day period post-release in order to insure that linkages to community providers occur.
  7. Participates in team delivered care coordination services across multiple settings (e.g., hospitals, jails, treatment facilities, transitional living facilities and shelters, and family homes).
  8. Demonstrates the ability to respond effectively to stressful situations.
  9. Accurately evaluates each client for present state and need, developing a diagnosis and recommending appropriate service(s) (including interviewing client and accompanying family or friends, providing crisis intervention, and arriving at treatment recommendations).
  10. Interact with patients, family, and support systems to reduce barriers and promote progression of care.
  11. Collaborates with medical staff and community-based agencies regarding evaluation, recommendation and referral disposition. (Includes DHS, JCS, law enforcement and other Hillcrest departments).
  12. Using theoretical knowledge of psychopathology and treatment techniques that are applied in a manner that benefits the client to provide assessment, counseling, information and referral, advocacy services, and other resource assistance as indicated
  13. Performs all aspects of patient/client care in an environment that optimizes patient/client safety and reduces the likelihood of medical/health care errors. Provides a safe environment for patients/clients and staff, including being aware of and documenting unusual incidents.
  14. Successfully completes initial assessment which includes completing treatment plans which summarize assessments and links patient/clients to the appropriate level of care no later than after the first therapy session. Plans contain goals that are specific, measurable, personalized, and achievable. Diagnosis/MSE and psychosocial within three sessions.
  15. Documents individual case notes within 48 hours of each session into clients' Electronic Health Record as required for the medical record and third party payer sources. Demonstrates appropriate levels of software utilization to support documentation requirements, agency policy and procedures, and quality service delivery.
  16. Correctly utilizes required outpatient forms and maintains accurate and orderly case records, including necessary EHR documentation and Quality Metrics reports.
  17. Completes or makes an earnest attempt to complete patient follow-up phone calls within 72 hours of referral from another EIR Care Coordinator. Responds to "time Sensitive" patient care coordination questions within 24 hours during established working hours. Responds to "time Urgent" patient care coordination questions within 90 minutes during established working hours. Phone calls and other pertinent information are documented according to agency and region guidelines.
  18. Communicates with relevant treatment teams in the community to enhance the success when discharged back to the community. Is knowledgeable of facility and community resources. Provides assistance with linkage to necessary resources in the community (entitlements, employers, housing, etc.).
  19. Maintains a good working relationship within the department and with other departments. Communicates appropriately and clearly to the Lead Therapist, co-workers, medical staff, and other professional contacts.
  20. Treats patients/clients and their families with respect and dignity. Is emotional and mentally stable to work with the program's clientele, model self-care skills and assist patient/client in meeting his/her goals.
  21. Demonstrates knowledge of and appropriate use of informed consent, confidentiality and client's rights. Obtains legal authorization to release or obtain information pertinent to each individual case.
  22. Participates in staff meetings and case conferences. Reviews client progress with other treatment providers as needed.
  23. Serves as a mental health resource to the agency and the community. Represents Hillcrest's interest in community groups at the discretion of the Lead Therapist or Division Director and shares information as requested.
  24. Increases skill level through continuing education and work with diverse clients.
  25. Identifies gaps in care and begins process to close gaps by assisting scheduling of follow up appointments with primary care provider, psychiatric provider, substance abuse providers, and participates in a warm hand off with the Eastern Iowa Care Coordinators.
  26. Submits Written Quality Metric Reports Weekly per agency and region guidelines.
  27. Complies with facility's policies and procedures and safety management.
  28. Follows the ethical standards of the agency and of licensing board. Keeps current with advances in the therapy field through continuing education, seminars/conferences, professional societies and journals, etc. and keeps up to date with licensure renewal.
  29. Demonstrates an understanding of PCOMS and utilizes PCOMS when applicable with the majority of clients served in therapy.
  30. Completes other duties as necessary and assigned by Program Director and/or Chief Executive Officer.
