Shifts Available 

(1) Fulltime 12 hour shifts, 7PM-7AM, 3 shifts per week, every other weekend, 72 hours per pay period
*$0.50 extra pay per hour from 7pm-11pm and $1.00 extra pay per hour from 11pm-7am for each shift*

Job Purpose:  Subacute care facility for persons with serious and persistent mental illness or subacute care facility means an institution, place, building, or agency with restricted means of egress providing subacute mental health services for a period exceeding twenty-four consecutive hours to persons in need of the services.  Crisis stabilization residential services are short-term services provided in facility-based settings of no more than 16 beds.

The goal of CSRS and subacute is to stabilize and reintegrate the individual back into the community. Crisis stabilization residential services are designed for voluntary individuals who are in need of a safe, secure environment less intensive and restrictive than an inpatient hospital. Crisis stabilization residential services have the capacity to serve no more than two individuals at a time. The crisis stabilization residential services at Hillcrest are provided for adults aged 18 and older.


Regulatory Requirements:

  • A bachelor's degree with 30 semester hours or equivalent in a human services field (including, but not limited to, psychology, social work, nursing, education); a law enforcement officer; an emergency medical technician (EMT); a certified peer support specialist; a certified family support peer specialist, or a registered nurse)
  • 3 years of experience in a mental health setting.
  • 30 hours of crisis and mental health in-service training (provided by Hillcrest)
  • Must hold a valid driver's license, have access to a reliable vehicle, and have that vehicle insured. Agency driver required.
  • National Career Readiness Certificate preferred.

Demonstrates Competency in the Following Areas:

  1. Provides supportive counseling with intervention, observation, and supervision that includes 15 minute checks for the first 24 hours, and 30 minutes for the next 24 hours and then hourly checks thereafter.
  2. Develops measurable goals and establish treatment plan goals and crisis plans based on areas of need when applicable as instructed by supervisor. Review stabilization daily with resident. Crisis plan as needed.
  3. Completes related client paperwork and records data in electronic health record (EHR) with accuracy. Completes shift documentation on each resident by the end of the shift.
  4. Works with residents to maintain the residence on a daily basis including chores, facility upkeep and other light housekeeping duties.
  5. Discharge planning begins within 24 hours of admission at crisis center or within 6 hours at subacute.
  6. Completes incident reports and other required paperwork to ensure the program meets the standards. Notifies on call nurse of any incident and supervisor of major incidents.
  7. Functions an effective team member and represents the agency in a professional manner. Works closely with Hillcrest's Mobile Crisis Outreach (MCO) counselors, staff from other HFS programs and other area health providers to implement and document treatment needs assessments, treatment plan, and progress reviews as part of continuous quality improvement activities.  
  8. Upon admission to the crisis facility, reviews policies and rules of house with client including client orientation checklist, inventory of belongings and following bedbug procedures of any clothing items (dryer: High heat)
  9. Reviews with each resident his or her crisis plan.
  10. Transports residents to scheduled appointments as necessary.
  11. Consults with other community agencies and significant others involved with the resident to live in the least restrictive setting.
  12. Advocates for the consumer/client rights and choices in obtaining services.
  13. Assumes other duties as assigned by the Crisis Stabilization Unit Supervisor or Lead Social Worker.
